When you upload your avatar the system places it in a file directory and renames the file to the same name as the old file was renamed to. When you load the page again your browser is given the same file name so it does not download the file again rather it gets the copy in the browser cache. You must use the Refresh button or icon to reload the new image. I suspect the new file did upload just fine and your browser was pulling the cache copy for display so you saw the old image. Try to upload the file again and the refresh your browser.

I've been aware of the problem for more than a month, what I have done that works, is to remove the old image and wait a day and upload the new image.
